Guide to FinalForms
Parent Registration
- Navigate to the HHSC FinalForms site
- Under the Parent icon, click New Account
- Type your name, date of birth, and email, and then click Register
- Check your email for a FinalForms email, and click Confirm Your Account in the email text
- Note: You will receive an email within two minutes prompting you to confirm and complete your registration. If you do not receive the email, check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com for further assistance
- Create your new FinalForms password and click Confirm Account
- Click Register Student for your first child
Prerequisites
Before you get started, make sure you have the following:
- Basic medical history and health information
- Doctor and dentist contact information
- Insurance company and policy number
- Hospital preference
Registering Students
Registering Your First Student
Note: If you have followed the prior steps, you may skip to step 3.
- Navigate to the HHSC FinalForms site
- Click Login under the Parent icon
- Click Register Student
- Type in the student's Legal Name and other basic information, and click Create Student
- If your student plans to participate in a sport, activity, or club, please click the checkbox for each. Click update after making your selection(s)
- Note: Selections can be changed at any time until the registration deadline
- Complete each form and sign your full name (i.e., 'John Smith') into the Parent Signature field at the bottom of the page. After signing, click Submit Form and move on to the next form
- When all forms are complete, you will see a 'Forms Finished' message
- Important: If required, an email will automatically be sent to the email address that you provided for your student prompting him/her to sign required Student forms
Registering Additional Students
To register additional students, log in to your account and click My Students. Please repeat steps 3-7 for each additional student.
Updating Information
To update information, you can log in at any time and click Update Forms to update information for any of your students.
