FinalForms (Physicals)

In-house high school physicals will begin at 8:30 AM on May 1, 2025. Athletes will be called to the Athletics Office by grade. Students with early dismissals should pre-arrange a time with Mrs. Moffatt.

If you cannot attend, you must see your own doctor. If you are called down during lunch, eat first, then immediately report to the physicals station (pool hallway).

Please wear casual athletic-type clothing (shorts, sweats, leggings--NO dresses or skirts).

Beginning April 14, 2025, turn in the Physical PPE packet with completed parent and student signatures to the HS Athletics Office with cash or check made out to "HHHS" in the amount of $30. Credit card payments may be made through Eventlink.

In-house middle school physicals will begin at 8:30 AM on May 22, 2025. Athletes will be called down by grade. Students with early dismissals should pre-arrange a time with Mrs. Patterson.
